No reservations will be held until this office has received a completed form, with a main office company check, money order, cashier's check or cash (do not send cash through the mail) for the full amount of the class fee ($200.00). Company checks from an agent's company account or personal checks will not be accepted, and will be returned. No reservation will be held. There will be a $20.00 fee for returned checks. Reservations must be made ten (10) working days before class date. Cancellations or transfers must be made no later than 5 days before class date. If you want written confirmation of your class date, enclose a self-addressed stamped envelope with your registration form. Credit for this class will not be given to those who are late, or for those who leave class early. Bring a picture ID to show when you check in. This should be your Arkansas driver's license or Arkansas state ID.

Class will be held on the dates listed below, in one day - beginning at 8:00 a.m. and ending at 5:00 p.m. Classes cannot be split. To receive credit for attending class, attendance must be all day.

All changes must be made five (5) working days in advance. Anyone missing their reserved date without notice will not be able to transfer payment to another date. There are NO REFUNDS, only credit can be given credit to companies for future classes.
